1. Text of Consent

In the meaning of regulation § 5 para. 2 of the Personal Data Protection Act no. 101/2000 Coll. On, in the latest from of the regulations (hereinafter also referred to as “the Personal Data Protection Act”) and in accordance with Art. 6, para.1, point a) of regulation 2016/679 of the European Parliament and Council (EU) of April 27, 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and on the cancellation of regulation 95/46/ES (General Regulation on personal data protection), which came into effect on May 25, 2018

will be collected, retained and processed by the controller for these purposes:
to conclude a contract, to provide it to a third party, who in this case is Svitak Zdenek in order for this party to use them for the following purposes: webmaster
4. Duration for which the personal data will be retained
The controller will retain the data for as long as is necessary. At the latest, the controller will retain personal data for 3 years.
5. How will the personal data be processed and stored?
Personal data will be processed digitally on computers and through software.
6. The rights of the data subject, information and access to personal data
As a data subject I have the right to access my personal data, the right to correct or delete them, or to restrict their processing. I have the right to request information on which personal data are processed and the right to demand clarification in respect of the personal data processing.
Furthermore, I have the right to object to the processing, as well as the right to transferability, of the data (i.e. the right to obtain personal data related to me that I have provided to the controller in a structured, commonly used and digital format, see Art. 20 of the GDPR for details)
I have the right to withdraw my consent at any time without prejudice to the legality of the processing based on my consent provided prior to its recall if the processing is regulated based on Art. 6, para. 1, point a) or Art. 9, para. 2, point a). This means I do not have such a right in particular if the processing is necessary to meet legal obligations incumbent on the controller (especially obligations related to Legalization of proceeds from criminal activities).
As a data subject, I am aware that I can withdraw my consent with providing data by filling out the form on the website www.icmuh.cz or by sending an email to icm@icmuh.cz.
As a data subject, I have the right to contact the controller or the Personal Data Protection Authority (Úřad na ochranu osobních údajů) if there is any doubt about compliance with the personal data processing obligations
The controller declares that in processing the data there is no automated decision making, including profiling, as referred to in Art. 22, para. 1 and 4 of the GDPR.
The controller declares that personal data is not processed for the purpose of scientific or historical research or for the purpose of statistics.
The controller declares that data will be processed digitally via computers and computer software.
7. Final Provisions
As a data subject, I declare that I am aware of my rights in accordance with §12 and §21 of the Personal Data Protection and Rights Act under the GDPR.
I declare that all provided information is accurate and provided on a voluntary basis.
The controller declares that data will be collected to the extent necessary to meet the purpose and processing will only be with the purpose for which they were collected.
This consent is the free and conscious expression of the will of the data subject, which is the consent of the data subject to the processing of personal data.
